Can’t find your COVID-19 vaccination record?

‌‌If you were vaccinated at a NSW Health clinic, please complete the COVID-19 Vaccination Record Query Form.  Please enter your details carefully so we can match them against our records.

‌To complete the form, you will need the following information:

• Medicare card number, individual reference number and expiry date. If you do not have a Medicare card/number, you will need an Individual Healthcare Identifier (IHI).

  Note: If you do not have an IHI, please visit the Services Australia website.

• The name of the clinic/s where you were vaccinated and the date/s of vaccination for Dose 1 and/or Dose 2.
